If you wish to apply for a place at Trinity Sixth you must complete the following forms and return to the school. These forms are available on our Sixth Form option evening at 6pm on Thursday 8th November, and after that from reception. You will also be able to download and print the forms by following the link below.

Please fill in your name and then give the form to your religious leader for completion and signing.
SCHOOL REFERENCE FORM
Please fill in your name and ask a member of staff at your current school to complete and return to Trinity
The SUPPLEMENTARY FORM, together with the REFERENCE FORM if appropriate, should be returned to:

The closing date for returning forms to the school is 2pm on Friday 14th December 2012.
